Note: The English version of this agreement shall prevail in the event of any discrepancy between the English and Japanese versions.

Welcome to Utah-Intercultural, a program created to guide you on the path to personal growth and a bright future. As you embark on your journey, we recognize the importance of setting clear expectations and establishing a framework that promotes responsibility, respect, and adherence to the program rules.
By signing this contract and following the program rules, you take an active role in shaping your own success and fostering a nurturing educational environment. Remember, your choices today will pave the way for your achievements tomorrow.
Section 1: Purpose and Importance of the Contract
This contract serves as a mutual understanding between students and the school (Internexus Provo), fostering an atmosphere of collaboration and cooperation. It outlines the principles, expectations, and guidelines that will empower you to make informed decisions, act responsibly, and maximize your experience.
Section 2: Acknowledging Responsibilities
2.1 Personal Responsibility
By signing this contract, you acknowledge that you are responsible for your own actions, decisions, and the consequences that arise from them. This includes but is not limited to attending classes punctually, completing assignments diligently, and engaging in respectful and inclusive behavior towards peers, your host family, and faculty.
2.2 Academic Responsibility
You acknowledge the importance of prioritizing your education and committing yourself to academic excellence. This includes active participation in class, regular completion of assignments, and seeking help when needed.
Section 3: Homestay
3.1 Tips
The goal of the Utah-Intercultural Homestay Program is to help students have an immersive experience in American culture.
- Spend time with your host family as much as possible.
- Participate in activities such as going to the park, church, or grocery shopping.
- Try to speak with them frequently to practice your English.
3.2 Policies
To ensure a positive experience, students must follow the rules below. Failure to comply may result in removal from the Utah-Intercultural Homestay Program.
- If you arrive home late, inform your host family and provide the name and phone number of the person you are with.
- Help maintain the cleanliness of the home and yard.
- Keep your bedroom and bathroom clean and organized.
- Ask permission before inviting friends to the house.
- Alcohol is not permitted. The purchase or consumption of alcohol by persons under 21 is prohibited.
- Smoking, vaping, or tobacco use is not permitted for persons under 21.
- Follow the rules established by your host family.
- Provide your host family with your emergency contact information.
- If you feel sick, inform your host family immediately.
Section 4: Image Release Agreement
I hereby grant Utah-Intercultural permission to use my likeness in photographs, videos, or electronic images in its publications, including website entries, without payment or consideration.
I understand these materials become the property of the organization and may be used for promotional or educational purposes.
I confirm that I am 18 years of age or older, or that a parent or guardian has signed below on my behalf.
Section 5: Cancellation and Refund
- Up to 90 days before the start date: the school may charge a cancellation fee of 10%.
- Within 60 days before the start date: the school may charge 100% of the program cost.
- After the program begins: any unpaid balance must be paid immediately.
Section 6: Registration Agreement
I certify that the information provided in my application form is correct. I understand the program costs and agree to take a placement test. I also understand that I must have valid health insurance during my stay.
Section 7: Medical Clearance
I authorize qualified medical treatment if necessary and understand that I am responsible for medical expenses not covered by insurance.
Section 8: Privacy and Security Policy
Utah-Intercultural respects your privacy and protects personal information collected through this website.

Section 9: Dress Code
Participants must dress appropriately for classes and activities.
Seasonal Clothing
- Winter (January/February): bring appropriate clothing for cold and ice activities.
- Summer (July/August): bring clothing suitable for water activities.

- ホストファミリーとできるだけ多くの時間を過ごしてください。
- 公園や教会、買い物などの活動に参加してください。
- できるだけ英語で会話をしてください。
第4条 画像使用許可
Utah-Interculturalが写真や動画などにおいて私の肖像を使用することを許可します。
第5条 キャンセルおよび返金
- 開始90日前まで:支払額の10%がキャンセル料として請求される場合があります。
- 開始60日前以内:プログラム費用の100%が請求される場合があります。
- 開始後:未払い残高は直ちに支払う必要があります。
第6条 登録同意
申請フォームに記載した情報が正確であることを確認します。
第7条 医療同意
必要に応じて医療処置を受けることに同意します。
第8条 プライバシーおよびセキュリティ
Utah-Interculturalは個人情報の保護を重視しています。
第9条 服装規定
授業および活動では適切な服装を着用してください。
- 冬(1月・2月):寒冷地での活動に適した服装を持参してください。
- 夏(7月・8月):水辺の活動に適した服を持参してください。
